Ultima VI - The False Prophet IBM 5.25" version in nice shape. Including cloth map, 2 sets of game floppies and some inlays.
Note that I have not tested the game, since I don't have a drive for it, but I have no reason to think it doesn't work. The 2 sets of game floppies should also provide some extra certainty.
